Form to fill out, before attending to any of my dance classes:
- I understand that when I purchase a dance class (drop in or a package option), they are not refundable. Only 1 person can use the same package.
- I understand and I agree that if I buy a package for 14 classes, I will have to use them all by the end of my fall 2024 dance season. It starts on September 1st 2024 and ends on December 8th 2024.
- I understand that if I have a package and already confirmed my presence at the class, but had to cancel it at the last minute (within 24 hours of it) – they are considered as used. 3 days before the class, you will receive a message from me to confirm your presence. However, I understand that emergencies can happen. I give you 3 chances to do it without losing your class.
- I understand that a dance class is a physical activity that has a risk of injuries.
I agree to make sure to feel in a good physical and mental state before attending the dance classes by Alexandra Chiriac or AleDance community. During a dance class, I agree to be careful with my movements, analyze my physical state and inform Alexandra Chiriac about any discomfort that I might feel. I accept the risk of injuries and I release Alexandra Chiriac and the AleDance community from all claims, physical damages, causes or actions coming from my participation at a dance class.
- I agree to communicate with Alexandra Chiriac before or after a dance class for any concerns or special approach I need.
- I don’t have medical injuries that prevent me from taking a dance class.
- I understand that during a dance class, some girls and I can take videos and pictures that can be used on social media or for my dance classes promotions.I agree to be filmed/taken pictures . (If you don’t agree with this note, please still accept the form and privately message me about not wanting you to be on any media.I will make sure that you wont appear.)
- I agree to respect the classes, people around me. If there is any please refer to me for resolution and solutions.
